Are you sure you have the correct versions of glibc, etc? Was this all
apt-get installed or have you built anything by hand?

I'd purge it all and start again:

dpkg --force-depends --purge apache2 apache2-common
apache2-mpm-prefork apache2-utils apache2.2-common

dpkg --force-depends --purge php5-cgi php5-common php5-gd php5-mysql

apt-get install apache2-mpm-prefork libapache2-mod-php5 php5-common
php5-gd php5-mysql
